Programming Updates for July

Here’s what’s on in July:

Street Health and Harm Reduction programming:

  • The Kit Group takes place on Tuesdays from 1-3PM in the Board Room. To sign up, please call 416.537.0275, ext 271, or drop by the Harm Reduction Room on the first floor.
  • The next ID Clinic will take place on Wednesday July 15th from 10AM-12PM.
  • KAPOW takes place Fridays from 10-11:30AM in the Board Room. For more information, please call Victoria at 416.537.0275, ext. 361.
  • housing worker from West End Housing Help is available on Monday afternoons from 1-4PM. For more information, or to book an appointment, please call 416.537.2455 or speak to the first floor receptionist.
  • Naloxone Training: Do you use opioids? Get training about how to respond to an opioid overdose using Naloxone. Training takes 20 minutes, and you will receive a FREE Naloxone Overdose Response Kit. Drop by Parkdale CHC on Friday Thursday July 30th between 2-4PM in the Board Room.  For more information contact The Works 416-392-0520.

Senior’s programming:

  • The English Conversation Club takes place on Mondays from 10AM-12PM. There will be no club on Monday July 6th.
  • The Seniors Wellness Drop-In takes place from Tuesdays from 1-3PM. The Drop-In will be cancelled on Tuesday July 21st.
  • The Seniors in Action Advocacy Group will take place on Thursday July 16th from 10-11:30AM.

  • The Healthy Smiles Dental Clinic is open from 8AM-4PM Monday through Friday.
  • The West End Oral Health Clinic is closed for the summer and will reopen in September.
  • The Infant Hearing Program takes place every Friday from 9AM-4:30PM. For more information, please contact 416.338.8255.
  • The Naturopath Clinic takes places on Tuesdays from 12:00PM to 6:00PM. To book an appointment, please request the sign-up sheet from the first floor receptionist.  There is currently a waiting list of six months.  You may also visit the clinic Tuesday afternoons and ask if there are any cancellations.
  • Anonymous HIV testing is offered on Tuesdays, Thursdays and Fridays for members of the general public. Please call 416.537.2455 to book an appointment.

Diabetes Programming:

  • The Living Well Lunch Club will take place on Monday July 13th at 12:30PM.
  • The Gentle Exercise Class takes place on Fridays from 10:45AM-12PM. This group will not take place on Friday June 12th. For more information, call 416.537.2455.
  • Living Life Well with Diabetes is an eight week program led by our Diabetes Nurse Educator and Dietitian that aims to boost knowledge, skills, and confidence relating to diabetes. Topics include blood sugar management, how to understand medications, physical activity, healthy eating on a budget, goal setting and stress management. This program will run on Thursdays from 1:30-3:30PM from July 16th to September 3rd.
  • This month we will be holding a Community Worker Training on Diabetes Management. This one day workshop, led by our Diabetes Nurse Educator and Diabetes Dietitian, is
    geared towards boarding home workers, cooks and food service workers, and community workers who work one-on-one with clients. This session will take place on Friday July 10th from 10AM-4PM. Please register with Mel by Friday July 3rd.

5Ps programming:

  • Baby and Me takes place Mondays from 1:00 p.m. to 3:00 p.m. at 27 Roncesvalles, Suite 503. This program is for parents and caregivers of babies 6-12 months.
  • Mom and Baby Circle takes place Mondays from 10:00 a.m. to 12:00 p.m. This program is for parents and caregivers of babies 0-6 months.
  • Prenatal Nutrition and Support takes place Thursdays from 10:00 a.m. to 11:30 a.m. and from 1:30 to 3:00 p.m.
  • Childcare is available for registered participants with children aged 0-12 years.

Women’s Connection Programming:

  • The Postpartum Mood Disorders Support Group takes place on Tuesdays from 2-4PM. This group is for mothers who are suffering from PPMD who live in the west end of Toronto. Childcare is provided. For self-referral for intake and assessment regarding postpartum mood disorders, clients may call Isabel at 416.537.8222, ext. 305, or Grazyna at ext. 303.
  • Trauma 101: Healing from the Effects of Abuse and Trauma is a six week psycho-educational and skill-building series of workshops to help women who have experienced trauma better understand and cope with its impact. This workshop is for women who have experienced childhood sexual abuse, adult sexual assault, domestic violence, refugee trauma/torture. This workshop series takes place on Wednesdays from 5-7PM from May 27th to July 8th, with no session on July 1st.To register, please call Grazyna at 416.537.8222, x303.

Gardening Drop-In:

  • Stop by the garden in Parkdale CHC’s backyard to help plant, water, weed, or simply enjoy a green space with Greenest City staff and gardeners. No garden experience is require and everyone is welcome. Meet Monique inside of Parkdale CHC on Mondays between 11AM-12PM starting June 15th.

Closure notice:

  • Parkdale CHC will be closed on Wednesday July 1st for Canada Day.

  • The Parkdale Elder Abuse Awareness Community Walk is taking place on Monday June 15th. Meet in the Parkdale CHC lobby at 10AM. We will walk along Queen St. to Jameson Ave, ending at the corner of Queen and Dufferin Street. The walk will be followed by a community discussion. Show the world that you care!
  • Living Healthier to Live Longer is a group for seniors living at 20 West Lodge Ave who may be experiencing social or health problems or who would like to learn about how to live a healthier life. This program takes place at 20 West Lodge Avenue from 10-11:30AM on Thursdays. Registration is required.

Families and Newcomers programming:

  • The Peer Nutrition program is designed for newcomer women and their children and is hosted by Parkdale CHC, Toronto Public Health, and West Neighbourhood House.  It offers an opportunity to cook and share a nutritious meal, engage with peers on settlement challenges for women parenting, and other issues. This group takes place on Wednesdays from 9:30AM-12PM from May 13th to June 17th.

This program will run on Thursdays from 1:30-3:30PM from May 14th to June 18th.

5Ps programming:

Childcare, TTC and a light meal will be provided. The next session will take place on Wednesday June 17th (Note that this month’s session takes place on the third week).

  • Yoga and Mindfulness for Moms is a trauma-informed group that aims to teach participants stress management skills, self-awareness, and self-compassion through techniques such as deep breathing, relaxation, and mindfulness meditation. This group takes place on Fridays from 10:30AM-1PM from May 1st until June 5th in Suite 503, 27 Roncesvalles Ave. Childcare, snacks, and TTC tokens will be provided. Space is limited. Please register by calling 416.537.8222 ext. 307 (Kendra) or 305 (Isabel).
  • Building Skills: Peer Learning and Empowerment Program is a skills building program that takes place on Tuesdays from May 19th to June 30th from 10AM-1PM. Build on your strength and skills, self-confidence, communication, and group work skills. This group is open to all who identify as women. Space is limited! To register, please call Nancy at 416.537.8222, ext. 310.
  • Join us for a six week Wellness Workshop on Thursdays from 2-4PM from May 7th to June 11th. Learn how to improve your emotional and physical well being through techniques like expressive arts, yoga, mindfulness, and relaxation. Snacks and TTC tokens will be provided.   • Nobody’s Perfect is a free parenting program for parents and caregivers with children six (6) years of age and under to talk about child growth and learning, child behaviour, child health and safety, and parenting stress. Childcare, snacks, and books are included. TTC tokens will be provided (if needed). This group meets weekly in sessions of 8-10 weeks. This session will run on Mondays from 1-3PM from March 23rd to May 25th.

Counselling Programming:

  • Parkdale CHC is offering Anger Management sessions that are geared towards individuals are who mandated by the law to take Anger Management courses or are interested in joining the group for their own personal growth. Sessions run weekly on Wednesdays from 4:30-6PM. The next sessions will run from April 1st until May 20th and then May 27th until July 15th. The program and necessary materials are free of charge. Referrals are accepted from probation officers, physicians, or other professionals. Self-referrals are also accepted. To register, please call Elizabeth Guete at (416) 537 2455. ext 306.

Did You Know? May is Foot Health Month!

  • Chiropodists/Foot Specialists are primary care professionals who practice podiatric medicine and specialize in the assessment, management and prevention of diseases, disorders and dysfunctions of the foot.  Their role is to maintain and improve patient mobility, alleviate foot pain, provide health education, and thereby improve quality of life.  Chiropodists/Foot Specialists are an essential member of the inter-professional health care team, skilled in assessing the needs of their patients and of managing both the acute and chronic conditions affecting foot and lower limb function. As a primary care provider capable of independent clinical practice, a Chiropodist/Foot Specialist can offer independent medical referral and medical supervision to a diverse range of patients and can specialize in different practice areas. Parkdale CHC offers Chiropody Services for registered clients.

World Asthma Day

World Asthma Day is an annual event organized by the Global Initiative for Asthma (GINA) to improve asthma awareness and care around the world. World Asthma Day 2015 will take place on Tuesday, May 5, 2015 and the theme will continue to be “You Can Control Your Asthma.” GINA will also continue to use the sub-theme, “It’s Time to Control Asthma.” If you or your loved one has asthma, we encourage you to take a quit asthma control test by following the link:  http://www.on.lung.ca/page.aspx?pid=399

Celebration of the Childminding Training Program

Recently, Parkdale CHC completed an evaluation of our Childminding Program. The evaluation demonstrated that the program has been a great success and we look forward to continuing to run it.

Important Announcement from the Board of Directors

April 10, 2015

Dear Community Partners and Friends,

The Board of Directors is taking the opportunity to stop and reflect on how we can best continue to position PCHC as a leader in the health system and how we can ensure our sustainability as a Community Health Centre. We are on our way in this journey, and wanted to provide you with an update.

We are pleased that Shirley Roberts has been in the role of Interim Executive Director since February 1, 2015, and are confident that she will continue to keep PCHC on course. After a Request for Proposals was distributed in February, we have hired the consulting firm Health Innovations Group (Joe Bornstein, Amanda Pieris and Preeti Nigam) to assist us with an environmental scan, consultations, shortlisting innovative approaches or practices, and the development of a governance toolkit that could be used by other CHCs in the Toronto Central LHIN. Health Innovations Group (HIG) provides public sector consulting services to Canadian governments and health care organizations. We are appreciative that the Toronto Central LHIN has supported the Board’s interest in reflecting on sustainability.

HIG has begun their work in earnest, and in the upcoming weeks will be reaching out to gather feedback from you on the questions of sustainability, systems leadership, and the continuum of care. We do hope that you can participate in this community consultation process, and look forward to receiving your comments.

As indicated in our original letter, we are taking this time to pause and reflect. We are deeply interested in hearing from staff, partner organizations and other organizations in the sector and system. As part of our consultative plan, we are in touch with other stakeholders and are interested in compiling this information over the next few months. Our goal is to consult as widely as possible.

We understand that you may have questions or comments and we are keen on connecting with you in this collaborative process. 5Ps Programming:

  • Baby and Me takes place Mondays from 1:00 p.m. to 3:00 p.m. at 27 Roncesvalles, Suite 503. This program is for parents and caregivers of babies 6-12 months. Please note that there is a wait list for this program.
  • Mom and Baby Circle takes place Mondays from 10:00 a.m. to 12:00 p.m. This program is for parents and caregivers of babies 0-6 months. Please note that there is a wait list for this program.
  • Prenatal Nutrition and Support takes place Thursdays from 10:00 a.m. to 11:30 a.m. and from 1:30 to 3:00 p.m.
  • Fatherhood 101: Dads Drop-In is a drop- in for fathers with children 0-3 years of age that takes place on the last Wednesday night of the month from 6-7:30pm at 27 Roncesvalles Ave, Suite 503. Childcare, TTC and a light meal will be provided. The next session will take place on Wednesday April 29th. 
  • Childcare is available for registered participants with children aged 0-12 years.

Senior’s Programming:

  • The English Conversation Club takes place on Mondays from 10AM-12PM. There will not be a session on Monday April 6th.
  • The Seniors Wellness Drop-In takes place from Tuesdays from 1-3PM. There will not be a session on Tuesday April 21st.
  • The Seniors Actively Socializing Walking Club meets every Tuesday at 11:30AM. Enjoy the weather and be active!

Families and Newcomers Programming:

  • Child Minding Training takes place on Wednesdays until May 27th from 3-5PM at Parkdale Intercultural Association.
  • Nobody’s Perfect is a free parenting program for parents and caregivers with children six (6) years of age and under to talk about child growth and learning, child behaviour, child health and safety, and parenting stress. Childcare, snacks, and books are included. TTC tokens will be provided (if needed). This group meets weekly in sessions of 8-10 weeks. This session will run on Mondays from 1-3PM from March 23rd to June 1st.

Health and Dental Programming:

  • The Healthy Smiles Dental Clinic is open from 8AM-4PM Monday through Friday.
  • The West End Oral Health Clinic takes place on Wednesday and Thursday from 4:30-7:30PM on April 1st, 2nd, 8th, and 9th. The clinic will also be open on Saturday April 18th from 11AM-3PM.
  • The Infant Hearing Program takes place every Friday from 9AM-4:30PM. For more information, please contact 416.338.8255.
  • The Acupuncture Drop-In takes place on Wednesdays from 10:15-11AM from March 11th to April 15th. For more info call Norma at 416.537.0275, ext. 233.
  • The Naturopath Clinic takes places on Tuesdays from 12:00PM to 6:00PM. To book an appointment, please request the sign-up sheet from the first floor receptionist.  There is currently a waiting list of six months.  You may also visit the clinic Tuesday afternoons and ask if there are any cancellations.
  • Anonymous HIV testing is offered on Tuesdays, Thursdays and Fridays for members of the general public. Please call 416.537.2455 to book an appointment.
  • Support to Quit is an eight week smoking cessation program that helps individuals quit smoking. Three weeks of optional support are available after the completion of the program. Participants may qualify for FREE Nicotine Replacement Therapy, such as gum, the patch, lozenges, or inhalers. This program will run on Thursdays from 2-4PM from February 5th until April 9th. Please pre-register with Shirley at 416.537.0275, ext. 302.

Diabetes Programming:

  • The Living Well Lunch Club will take place on Monday April 13th at 12:30PM.
  • The Gentle Exercise Class takes place on Fridays from 10:45AM-12PM. For more information, call 416.537.2455.
  • The Insulin Management Group is a five week workshop that will run on Thursday March 19th and 26th, April 9th, 16th, and 23rd from 1:30-3:30PM.  This group is targeted towards individuals with type 2 diabetes who are currently using insulin, will be using insulin, or would like to learn more. Counselling Programming:

  • Parkdale CHC is offering Anger Management sessions that are geared towards individuals are who mandated by the law to take Anger Management courses or are interested in joining the group for their own personal growth. Sessions run weekly on Wednesdays from April 1st until May 20th from 4:30-6PM. The program and necessary materials are free of charge. Referrals are accepted from probation officers, physicians, or other professionals. Self-referrals are also accepted. To register, please call Elizabeth Guete at (416) 537 2455. ext 306.

Closure Notice:

  • Parkdale CHC will be closed on Friday April 3rd and Saturday April 6th for Good Friday. We will be open for regular hours on Monday April 6th.

Upcoming May Programming:

  • Yoga and Mindfulness for Moms is a trauma-informed group that aims to teach participants stress management skills, self-awareness, and self-compassion through techniques such as deep breathing, relaxation, and mindfulness meditation. This group takes place on Fridays from 10:30AM-1PM from May 1st until June 5th in Suite 503, 27 Roncesvalles Ave. Childcare, snacks, and TTC tokens will be provided. Space is limited.
