Community Worker Training on Diabetes Management

The Diabetes Education Program at Parkdale CHC will be running a one day workshop on diabetes management that is geared towards boarding home workers, cooks and food service workers, and community workers who work one-on-one with clients. The workshop will take place on Friday February 10th from 10AM-4PM at our main site at 1229 Queen Street West. Click here to view the workshop poster.

Topics discussed include:

  • General diabetes information
  • Hypo/hyperglycemia signs and symptoms and management
  • Nutrition and diabetes
  • Emergency care information
  • Long term complications

Space is limited and pre-registration is required by Friday February 3rd. Lunch will be provided. To register, please email melissa.hergott@pchc.on.ca and specify your name, job title, organization, and any dietary restrictions.

We want to know what you think about how we are doing and what we could do better.  This survey is completely anonymous and confidential and your choice to participate in this survey will in no way affect the services you receive at PCHC. The survey should take no more than 5-10 minutes to complete. To complete the survey, you can:

  • Download and print a PDF copy and bring the completed copy with you to your next visit.

  • Request a hard copy of the survey from staff at the Health Centre at your next visit. Surveys are available at reception.

  • The deadline for survey submissions is March 15th, 2017.
