Welcome to Parkdale Queen West Community Health Centre!

Effective April 1, 2017, Parkdale Community Health Centre and Queen West - Central Toronto Community Health Centres have voluntarily merged to become one centre. Our new name is Parkdale Queen West Community Health Centre.

This is an exciting time for community health services in mid and west Toronto. By coming together we have a strengthened foundation and have created enhanced opportunities to meet the emerging needs of our clients and communities.  Together we will continue to provide our clients with better, more equitable access to care and a broader range of services located close to home.

The sites will continue to provide their current services and programs and the providers delivering those services will remain the same.  Clients will also continue to access services at the sites in the same manner they do currently.

The sites will not be integrated.  The types of services and programs we provide are local and need to remain accessible to clients in both the Parkdale and Queen West neighbourhood.

A service planning process, with input from clients and staff, will examine how to evolve programs and services across the integrated organization.  The planning process will focus on how best the teams can work together, what services make sense to continue separately, and how best to support the clients we serve and those needing our services.

As we work to build a truly integrated health care system, each of our three sites is committed to delivering the same great care our clients and community partners have come to expect from their local community health centre.

If you have questions or feedback, please contact us at Info@ctchc.com.
