This month the Parkdale main site is hosting two special events for World Diabetes Day and Fall Prevention Awareness Month:

  • KAPOW takes place Fridays from 10-11:30AM in the Board Room. For more information, please call Victoria at 416.537.2455, ext. 1361. This group will be cancelled on Friday November 17th.

Population Health Programming:

  • The ID Clinic will take place on Wednesday November 15th.

Senior's Programming:

  • The Seniors Wellness Drop-In takes place on Tuesdays from 1-3PM. 
  • Seniors in Action will take place on Monday November 20th from 1-2:30PM.
  • Fit for Life is a pole walking group for participants aged 55 and above. Join us for pole walking, pole strengthening, balance, and stretching. This program takes place on Wednesdays from 10:30-11:30AM. Please wear comfortable clothes and proper footwear for indoor and outdoor walking. Group registration is limited to 10 participants.
  • Living Life to the Full is a program for caregivers over the age of 55 and teaches new skills for coping with life's challenges. Use the principles of cognitive behavioural therapy to learn more about mental well-being and how to make small changes to feel happier, less stress, and more in control. This program runs from 10:30AM-12:00PM on Mondays starting September 18th. Space is limited. Registration is required.
  • Chronic Disease Self-Management Program is a program for anyone living with a chronic condition or providing care and support to someone living with long lasting chronic condition. Participants will learn more about positive thinking, goal setting, problem solving, physical activity and healthy eating, decision making skills, and communication skills. Participants will receive TTC tokens, a relaxation CD, Living a Healthy Life with Chronic Pain (book), and $10 grocery card (must attend four or more sessions). The program runs on Thursdays from 9:30AM-12PM from October 5th to November 9th.
  • Low Impact Aerobic Fitness takes place every Tuesday and Thursday from 10:00-11:00AM at 201 Cowan Avenue.

Diabetes Programming:

  • The Living Well Lunch Club will take place on Monday November 20th at 12:30PM.
  • Living Life Well with Diabetes is an eight week introductory workshop that teaches participants about blood sugar management, medications, healthy eating on a budget, physical activity, coping with stress, and more. The program runs Thursdays from 1:30-3:30PM from October 5th to December 7th.
  • World Diabetes Day is an all day health fair that will be held on Friday November 17th from 10AM-3PM on the first floor. We'll have information booths, special presentations, raffle prizes, and more. Lunch will be provided by the West End Food Co-op. Wear blue to show your support!

  • Prenatal Nutrition and Support takes place Thursdays from 10:00 a.m. to 11:30 a.m. and from 1:30 to 3:00 p.m.
  • Fatherhood 101: Drop-In for Dads is a drop-in for dads-to-be and dads with children 0-3 years of age. This group will take place on Wednesday November 8th and 22nd from 6pm – 7:45pm at 27 Roncesvalles Ave, Suite 503. Childcare, TTC tokens, and a light meal will be provided.
  • Childcare is available for registered participants with children aged 0-6 years. If you would like to register in any 5Ps programs, please call 416.537.8222, ext. 3502 or 3505.

Women's Connection Programming:

  • The Postpartum Mood Disorders Support Group takes place on Tuesdays from 2-4PM. This group is for mothers who are experiencing symptoms of PPMD who live in the west end of Toronto. Childcare is provided. For self-referral for intake and assessment regarding postpartum mood disorders, please call 416.537.8222.

Notice of Implementation of New Phone System

Date: October 2, 2017

To: Clients and Stakeholders

From: Parkdale Queen West CHC (Parkdale Site)

RE: IMPLEMENTATION OF NEW PHONE SYSTEM NOTICE

As part of our ongoing integration work the Parkdale site phone system will be upgraded to be fully aligned with all the sites. The new system is expected to be completed and become live on Monday, October 16, 2017.

We hope for a smooth transition and that medical secretaries, receptionists and the rest of the staff are well oriented to the new system by Oct 16th.  We will do our best to promply assist you when you call.  We also ask for your patience and understanding, during this transitional period, as we can expect some small technical issues that may need to get sorted out.   Callers may experience longer wait times, calls may be lost or cut off. Thus, we would like to sincerely apologize in advance for any inconvenience that this may cause if it happens. If for some reason you’re unable to reach us by phone and you need to reach the Centre urgently,  please send us an email in our general email address @ info@pchc.on.ca.

  • Join us for a three week Spice Workshop! For thousands of years, herbs and spices have served as currency, medicine and, most importantly, the geographical “flavor print” of meals. Their magic can infuse ordinary ingredients with vibrant flavors. In these sessions we’ll take simple salads, grains, protein and dessert recipes, and use herbs and spices to transform them into a flavourful adventure! We will teach you how to prepare foods that are healthy and full of flavour using simple and delicious herbs and spices. We'll also go on a field trip to buy your own spices and see many spices of the world all found in this one Toronto shop! This group takes place on October 13th, 20th, and 27th from 11AM-2PM.

  • Baby & Me takes place Mondays from 1:00 p.m. to 3:00 p.m. at 27 Roncesvalles, Suite 503. This program is for parents and caregivers of babies 6-12 months. Please call 416.537.8222, ext 3502, for more information.
  • Prenatal Nutrition and Support takes place Thursdays from 10:00 a.m. to 11:30 a.m. and from 1:30 to 3:00 p.m.
  • Fatherhood 101: Drop-In for Dads is a drop-in for dads-to-be and dads with children 0-3 years of age. This group will be closed for the month of August and will re-open in September. The first session will be on Wednesday September 13th from 6pm – 7:45pm at 27 Roncesvalles Ave, Suite 3503. Childcare, TTC tokens, and a light meal will be provided.
  • Childcare is available for registered participants with children aged 0-12 years. If you would like to register in any 5Ps programs, please call Yohama at 416.537.8222, ext. 3502 or Stacia at ext. 3505.
  • All 5Ps programs will be closed from Monday July 31st through to Friday August 11th inclusive. All programs will re-open on Monday August 14th

  • The Postpartum Mood Disorders Support Group takes place on Tuesdays from 2-4PM. This group is for mothers who are experiencing symptoms of PPMD who live in the west end of Toronto. Childcare is provided. For self-referral for intake and assessment regarding postpartum mood disorders, please call 416.537.8222.

Programming Updates for July

Here's what's on in July:

Harm Reduction Programming:

  • The Kit Group takes place on Tuesdays from 1-3PM in the Board Room. To sign up, please call 416.537.2455, ext 271, or drop by the Harm Reduction Room on the first floor. 
  • KAPOW takes place Fridays from 10-11:30AM in the Board Room. For more information, please call Victoria at 416.537.2455, ext. 361.
  • The Harm Reduction Program will be open for regular drop-in hours.

Population Health Programming:

  • Shirley Hepditch, Case Worker, has drop-in hours on Monday and Friday from 10AM-12PM and Tuesday from 2-4PM. The drop-in will be cancelled on July 21st and 24th. 
  • The ID Clinics for July and August have been cancelled. The Queen West site will be holding ID Clinics on Wednesday July 12th and Wednesday August 9th. Registration starts at 9:30AM. For more information, please call (416) 703-8482, ext. 100, or view their website.
  • housing worker from West End Housing Help is available on Monday July 10th and 24th from 1-4PM. For more information, or to book an appointment, please call 416.537.2455 or speak to the first floor receptionist.

Senior's Programming:

  • The English Conversation Club will not take place in July or August.
  • The Seniors Wellness Drop-In takes place on Tuesdays from 1-3PM. 
  • Fit for Life is a pole walking group for participants aged 55 and above. Join us for pole walking, pole strengthening, balance, and stretching. This program takes place on Wednesdays from 10:30-11:30AM. Please wear comfortable clothes and proper footwear for indoor and outdoor walking. Group registration is limited to 10 participants. 
  • Seniors Cooking Together: Join us to cook, learn, and share a meal together! Eligibility: seniors 65+. This program runs on Friday July 7th and 28th from 10AM-12:30PM. Registration is required. 
  • Strong and Steady Falls Prevention: Join us for a seven week program led by Ana Maria and Jennifer, Registered Physiotherapists, and Sandra, Health Promoter. This program will take place on Wednesdays from July 5th to August 16th from 3:30 to 4:30PM. Please wear loose clothing and proper footwear. Registration is required.

  • Familes and Newcomers programming will resume in the fall. 

Health and Dental Programming:

  • The Living Well Lunch Club will take place on Monday July 10th at 11:30PM. Note the earlier start time!
  • Plan Well, Budget Smart, Be Healthy is a program for individuals with diabetes or pre-diabetes. Learn how to plan menus, grocery shop, and prepare healthy meals during this five week program led by our Diabetes Nurse Educator and Diabetes Dietitian. This program will run on Thursdays from 1:30-3:30PM until August 3rd. There will not be a session on July 27th.
