Case Manager

Shirley Hepditch,Case Manager, provides the following services:

  • Information on internal and external programs and services
  • Information on how to navigate our health care and social services systems
  • Assistance with ODSP and CPP forms, letter writing, and obtaining ID
  • Coordinates the monthly ID Clinic
  • Access to resources (e.g. food and transportation)
  • Smoking cessation counselling
  • New intakes from Social Service office

Shirley is available via telephone at 416.537.2455, ext. 1302. Her drop-in hours are Monday and Friday from 10AM-12PM and Tuesday from 2-4PM.
