Primary Health Care
Parkdale Community Health Centre offers a range of primary health care services for residents of all ages and genders who live in our catchment area. Our goals are to work with the whole person to treat and manage acute and chronic health conditions. PCHC also strives to provide information needed for individuals to make informed decisions about their health care.
We give priority to people who face barriers to health care such as language differences, are non-insured (do not have OHIP), seniors, are pregnant, homeless or under housed, live with mental health and substance use issues, and who have low income.
Nurses, Nurse Practitioners and Physicians provide one-to-one clinical services to people who may face multiple barriers when looking for health care services. Working within an interprofessional team of social workers, physiotherapist, chiropodists, dietitians, diabetes specialists, psychiatrists and naturopaths, health promoters, and harm reduction and outreach workers, we strive to treat the whole person. We provide a range of services including:
- Physical examinations
- Treatment and management of acute and chronic health conditions
- Ongoing follow-up for health care
- Pre and post natal care
- Reproductive health care
- Birth control
- Well baby visits
- Health teaching
- Immunizations and flu shots
- Referrals to social support services
- Limited home visits
In order to receive primary health care services at Parkdale Community Health Centre, you must:
- Live in the neighbourhoods bounded by Bloor to the north, Lakeshore to the south, Parkside to the west and Dovercourt to the east.
- Fill out an application form to register for service. Download an application form here.
- Be a registered client to access service (see exceptions below).
- Bring proof of address before being seen by a physician or a nurse or nurse practitioner (except if homeless).
- Please note: you cannot already be seeing another physician elsewhere.
If you are uninsured (do not have OHIP), you can receive services if:
- You are a landed resident and are in the three month waiting period for OHIP.
- You do not have status and have lived in Canada for six months or more.
- You are receiving Interim Federal Health Funding.
- You are homeless/under housed and have lost your health card.
You are NOT eligible for primary health services if you:
- Are a visitor or a tourist.
- Are over 32 weeks pregnant.
Our hours of service are:
Monday to Thursday: 9:00 AM - 8:00 PM
Friday: 9:00 AM – 5:00 PM
Saturday: 10:00 AM – 1:00 PM
